Welcome to the Clinical Data Repository (CDR)

By accessing or using the CDR system, you acknowledge and agree to the following terms and conditions. Please read them carefully.

  1. Intended Use:
    The CDR is designed to support authorized personnel, such as healthcare professionals, data managers, administrators, and researchers in the collection, storage, and retrieval of patient health data for purposes including, but not limited to, clinical care, health system management, reporting, and research (where authorized). Use of this system must comply with all applicable laws, regulations, policies, and organizational procedures governing patient data privacy and confidentiality. For research purposes, data should be accessed with a research protocol approved by a human health research ethics committee, as appropriate.
  2. User Responsibilities:
    Users must be authorized and properly credentialed to access the system.

    Users are responsible for maintaining the confidentiality and security of access credentials.

    All data accessed through the CDR must be handled in accordance with applicable privacy, security, and data protection regulations.

    Users must ensure that data entered into the system is accurate, complete, and timely to the best of their ability.
  3. Restrictions on Use:
    Unauthorized access, distribution, or use of data is strictly prohibited.

    Users may not use the CDR for personal, commercial, or non-authorized research purposes.

    Attempting to bypass system security, tamper with data, or engage in any activity that compromises the integrity of the system is strictly prohibited.
  4. Data Privacy and Confidentiality:
    The CDR contains sensitive health information. Users must take all necessary precautions to ensure patient confidentiality is maintained at all times, and may only share information with authorized parties as permitted by applicable policies and legal frameworks.
  5. System Availability and Limitations:
    While reasonable efforts are made to ensure the availability and accuracy of the system, the CDR is provided “as is” without warranties of any kind. Where appropriate, you agree to verify the source and accuracy of data on which clinical decisions are based. The system administrators are not responsible or liable for any loss or damage arising from system unavailability, inaccuracies, or unauthorized use.
  6. Acceptance:
    By accessing and using this system, you acknowledge that you have read, understood, and agree to these terms and conditions.
